If your child is not assigned to Dr. Sneed your visit will be re-scheduled for a later date.On The Day Of Your VisitBe sure to bring your childs Immunization Record, Current Medical Insurance Card and a list of all the Current Medications (Prescriptions & Over the Counter) your child is taking (Be sure to write down the dose and how often it is given).Bring insurance forms/identification cardsUpon your first visit to our clinic, you will be asked to fill out our Patient Demographics Form.This one page health background form is now on-line for your convenience.
